There are many different types of
health insurance coverage.
Your responsibilities as a patient
depend on the contract between you and your insurance company
and whether or not Piedmont Ear, Nose & Throat Associates
(ENT-Specialists) has a contract with your insurance company.
ENT-Specialists has contracts with many but not all insurance
companies. Please check your provider manual to make sure
our physicians are included. Below is an alphabetical listing

Co-pays
The majority of insurance plans today require the patient
to pay co-pays for office visits. The co-pay amount is listed
on your insurance card. The co-pay is part of the contract
between the insurance company, the doctor, and the patient.
The doctor is required by the insurance company to collect
the co-pay from the patient. ENT-Specialists, therefore, requires payment
of your co-pay at the time of your visit, and it will be
collected when you check in for your appointment.
Insurance Filing
If ENT-Specialists has a contract with your insurer, ENT-Specialists will file
your insurance claim. If not, you will be required to pay
for your visit. Our staff will provide you with a claim form,
and it will be your responsibility to send the claim to your
insurance company. The insurance company will then reimburse
you. ENT-Specialists will file all insurance claims for surgical procedures
if the procedure is covered by your insurance plan.
