Due to the heavy call volume on Monday, you may want to call Tuesday through Friday for non-urgent questions or appointments.

If an emergency or serious concern arises after office hours, you can always reach a physician by calling our Winston-Salem office number and leaving a message with our answering service.

Please make an appointment before coming in for all routine visits. Be sure to bring any insurance cards or relevant referral forms so our staff may better serve you. In addition, please bring any x-rays or other test results that may be helpful to our physicians.

New patients are asked to come in 15 minutes before their scheduled appointment time. If you have received a new patient packet, please complete the forms enclosed in the packet and give them to the receptionist at check-in.

By law, minors must be accompanied by their parent or legal guardian. If child being brought by someone other than legal guardian, please send a letter authoriting person bringing child to be treated.

We want you to be comfortable in our office. In the event that an emergency or unforeseen situation sets us behind schedule, we ask for your patience and understanding. You will receive the same care and attention as those before you.

Fees and Payments

To help keep costs down, we ask that you pay for our services or any deductibles or co-payments at the time service is rendered. If this is a hardship, we are happy to work out a payment schedule that suits your needs. We accept all major credit cards, personal checks, and cash.

Please check insurance info for a list of insurance companies with which we participate. If your insurance company is not on this list, you may be eligible for out-of-network benefits. Check with your insurance company representative or dial the 800 number on the back of your insurance card.
